This talk integrates two studies across five papers examining how sex and gender shape autism diagnosis, focusing on two central findings supported by large-scale quantitative and qualitative analyses.
First, sex-based measurement invariance analysis of ADOS-II data from the National Database of Autism Research reveals consistent sex differences in symptom scores and indicates that several items function differently for males and females, suggesting that commonly used diagnostic tools may reflect a male-oriented measurement structure.
Second, qualitative interviews with autistic adults illustrate how gendered expectations, masking behaviors, and clinician bias contribute to delayed or missed diagnoses.
Together, these studies highlight the interplay of structural and social factors in shaping autistic identification and underscore the need for gender-informed diagnostic practices.
